Intake Coordinator – Licensed Nurse

As an LVN Intake Coordinator you will facilitate the coordination of referrals and communications, ensuring referrals are thorough and complete. Entering all information completely and accurately into patient management system, while functioning as facilitator between field clinicians and management team.
What you’ll do as a LVN Intake Coordinator:
- Facilitate the referral intake process, coordinate care with the interdisciplinary group, the patient, their family, and the referral source
- Verifies each patient’s payor source and maintains and tracks insurance authorizations and initial certifications
- Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group
- Ensures that all personnel are informed of patient’s hospitalization and follow-up with patients admitted to other hospitals
- Reviews and Approves documentation
- Handles and coordinates incoming calls related to patients, physicians, referrals and agency services regarding physician orders, patient questions and patient referrals
- QA all Admission to ensure all required documents are present
- Assists with staffing/scheduling activities, soliciting input from managers
- Provides follow-up for hospitalized patients, ensuring subsequent agency referral
- Participate in coordinating care with Nurse Supervisors and patient interdisciplinary team
- Perform on call rotation as assigned per on-call schedule
- Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions
- Participate in implementing established education plans for patients and their families with common health problems and well-defined health learning needs
- Coordinate patient care as directed by providers, RNs, and policies, including referrals, transition visits, medications, procedures and follow up
- Telephonically obtain information on chief complaint and symptoms and accurately relays information to provider
- Collect data and performs “focused” nursing assessment of patients; assist in the evaluation of a patient’s response to treatment and identify patient’s needs; communicate findings to the provider in an accurate and timely manner
- Review the patient record, chart, reports (including laboratory and x-ray), and other pertinent information for patients prior to being seen by the provider and reports relevant information to the provider and/or RN
- Ensure patient confidentiality (HIPAA) at all times and treat patients with courtesy and respect
- Performs other job-related duties as assigned
To be a success as a LVN Intake Coordinator:
- Experience working with the intake and scheduling functions within a licensed home health company
- Demonstrates proficiency in advanced office computer applications
- Excellent customer service skills Excellent organizational, communication
- Current TX LVN license
- Promote Agency philosophy and administrative policies to ensure quality of care Demonstrate commitment, professional growth and competency
- Current BLS/CPR Certification
- Valid TX Driver’s License
- High level of clinical knowledge in home health care is highly preferred
